Description and usage:

Hydroxyethyl starch (HES/HAES) is a nonionic starch derivative. It is one of the most frequently used volume expanders under the trade names.  HES is a general term and can be sub-classified according to average molecular weight, molar substitution, concentration, C2/C6 ratio and Maximum Daily Dose.

Hydroxyethyl Starch HES 200/0.5,  Hydroxyethyl Starch HES 130/0.4 is a synthetic colloid volume expander which is utilized to maintain vascular volume in animals with circulatory shock. Hydroxyethyl Starch HES 200/0.5,  Hydroxyethyl Starch HES 130/0.4  is a modified branched-chain glucose polymer derived from amylopectin sources like potatoes, sorghum, or maize.

Hydroxyethyl Starch HES 200/0.5,  Hydroxyethyl Starch HES 130/0.4 are composed of more than ninety percents of amylopectin. It is a plasma voluma expander. Hydroxyethyl Starch HES 200/0.5,  Hydroxyethyl Starch HES 130/0.4 is used in the management of hypovolumic shock.

Indications:

Applications:

The main application field of Hydroxyethyl Starch (HES) is volume therapy, where it helps to improve the rheological properties of human blood. It is routinely used in emergency medicine to stabilize the cardiovascular system, or for shock treatment and blood replacement.

A large loss of blood volume, hypovolemia, can happen for a variety of reasons, for example bleeding, or severe burns. Considering hypovolemic shock can quickly become life threatening, it is important to replace the lost blood volume as fast as possible so that the patient's circulation keeps working. This is often done with colloidal infusions. Hydroxyethyl Starch is one of the most proven and trusted colloidal substances used in this group of plasma expanders.

Solutions with Hydroxyethyl Starch are well tolerated and safe and have good rheological properties. In the best formulations, Hydroxyethyl Starch HES does not accumulate in the plasma, is excreted at a predictable rate, and has a minimal effect on blood coagulation.

Colloids are used more often for resuscitation in the intensive care unit than crystalloids. Because of lacking data on hydroxyethyl starch 130/0.38-0.45, previous systematic reviews have been inconclusive about the benefits and harms of this colloid compared with other fluids. The most recent researches of three large trials comparing hydroxyethyl starch 130/0.38-0.45 with crystalloids in patients with sepsis calls for an updated systematic review to inform on the benefits and harms of this colloid in patients with sepsis, which is highly needed as fluid alternatives are available.

There is a reason to believe that the effects of hydroxyethyl starch 130/0.38-0.45 VS crystalloids or human albumin on all cause mortality, kidney injury, bleeding, and serious adverse events in patients with sepsis.

Increased mortality and/or severe renal injury requiring renal replacement therapy reported in critically ill adults, including patients with sepsis, receiving various HES preparations.

--Hemodilution

Hemodilution, resulting in dilution of blood components (For example, coagulation factors, other plasma proteins) and decreased hematocrit has been reported after using Hydroxyethyl Starch HES 200/0.5,  Hydroxyethyl Starch HES 130/0.4.

--Hematologic Effects and Coagulopathy

Administration of high dosages of Hydroxyethyl Starch HES 200/0.5,  Hydroxyethyl Starch HES 130/0.4 might end up in decreased concentrations of coagulation factors, other plasma proteins, and hematocrit. Depending on dosage administered, disturbances of blood coagulation beyond hemodilution effects also may happen, but it is very rare. Monitor coagulation parameters in people receiving prolonged therapy or as clinically indicated.
